- Street and house/apartment number (only this information)
- Email address (only one candidate per email; ensure you have access and receive notifications)
- Country code
- Phone number with country code
- Do you have valid visas or other markings in your passport?
- Are you a resident of a country other than your nationality or do you hold permanent residence in a different country?
- Tax identification number (required for verification by the Polish consul).
- Country and city where the visa will be processed.
